To download file, click on the file name below and when prompted click on Save.  Note where the file is saved on your computer as you will need this information to open the file.

To open file, go to the location of the file and click on Inventory Form.doc. 

Fill in the form and save (please add your last name to the beginning of the file name.)

Email form to gallery@tactilearts.org as an attachment indicating in the email which show this inventory applies to.
